Preadmission Screening and Resident Review (PASRR)
Overview
Preadmission Screening and Resident Review (PASRR) is a federally mandated program that is applied to all individuals seeking admission to a Medicaid-certified nursing facility, regardless of funding source.
Three Main Goals Of PASRR
- To identify people, including adults and children, with mental illness, and/or an intellectual or developmental disability or related condition;
- To ensure appropriate placement, whether in the community or the nursing facility;
- To ensure people receive specialized services for mental illness, and/or IDD, if eligible.

As the Local Intellectual & Developmental Disability Authority, Metrocare must screen every resident before admission to any Nursing Facility that accepts Medicaid, regardless of payor source. The PASRR Level 1 (PL1) Screening determines if an individual is suspected of having a mental illness or a related condition and qualifies for support services.

**Note: Families only need to fill out highlighted fields on the Screening Form. Families must select a facility.
- If the screening is positive, a PASRR Evaluation (PE) will be completed.
- If the screening is negative, the PASRR process ends for the individual.
A PASRR Evaluation (PE) is an extensive evaluation by PASRR staff to assess the individual’s skill level, health needs, diagnosis, and best treatment options. The PE will be completed within 7 to 14 days after the Level 1 Screening and service coordination will then begin.
Documents Requested During Evaluation:
- Health/Medical Information (Any Diagnosis, Medication History, Hospitalization Records)
- Supporting documentation of an intellectual or developmental disability; this can include but is not limited to the following:
- Special Education Testing (Full and Individual Evaluation (FIE) and/or Review of Existing Evaluation Data (REED))
- Any Psychological Reports and/or psychiatric records stating test results and diagnosis of the individual
- Any Other Legal Documents (Conservatorship Order, Letters of Guardianship, Adoption papers, Divorce Decree, Custody papers, etc.)
Based on the PASRR Evaluation, a Metrocare Service Coordinator will meet with your loved one in the nursing facility within 14 days to discuss services and treatment options. Services will begin within 20 days.
